Transition & Individual Transition Plans (ITPs)

This workshop provides an overview of Transition planning within the IEP process, including all Transition‑related meetings and requirements. Parents and guardians will learn what to expect as students move from 8th to 9th grade and how the Individual Transition Plan (ITP) guides students through the remainder of their IEP years. The session will also explain key components of Transition services, helping families understand timelines, goals, and how to support their student’s path toward high school, post‑secondary options, and adulthood.

Intended Audience: For parents and guardians of middle school students through transition‑age (12+)
